2009-2010 Membership Application

There are several ways to complete your Membership Application:

  1. Submit the application online and pay for membership fees online via PayPal, credit card, debit card or E-check.
     

  2. Submit the application online and mail a check (not the printed app) for membership fees to Treasurer Christy Garner, 8601 E. Hwy 78, Villa Rica GA 30180
     

  3. Print the blank application, complete by hand and mail the handwritten app with a check for $25 to Treasurer Christy Garner, 8601 E. Hwy 78, Villa Rica GA 30180.

If you want a copy of your completed membership application for your records (not to mail), click print before you submit your application.  Please answer every question on the application thoroughly.  If you submit your application online, you will be prompted to answer every question. To prevent delays in the approval process of your application, please answer the questions honestly and accurately.  If you complete your application by hand and mail it in, please respect our Membership Coordinator's time by remembering to thoroughly answer every question on the application.
